How to Apply Using Your Mobile
Here’s how seniors (or their family members) can apply:
1.You can download and install Ayushman App on Google Play Store and Apple App Store.
2.Report as or log on as such authorized representative or beneficiary.
3. Enter your mobile number, captcha and select a login option.
4. Enter the beneficiary Aadhaar details and state.
5. Proceed with eKYC by authenticating through OTP if the beneficiary is not traceable.
6. Fill and submit the declaration form containing all information required.
7. Confirm the mobile number through the OTP received.
8. Enter the correct category and PIN code of your area.
9. Include relatives’ details, if necessary.
10. Take the Ayushman card right away after authenticating and approving it.
