1.Individuals eligible for Scheduled Caste (SC) or Scheduled Tribe (ST).
2.People employed as daily wage workers.
3.People working in the unorganized sector.
4.Families falling under below poverty line (BPL).
5.People that are not already benefiting from any other government health schemes.
6.Minimum age: You must be at least 18 years old to apply.

How to get an Ayushman card: Easy 5 Step Process
1. Go to your nearest Common Service Centre (CSC)
Your first point of contact is your nearest Common Service Centre (CSC). These are the government designated centres that have been set up to help with basic citizens and citizen schemes.
2. Check your eligibility
Speak to the designated officer at the CSC Centre so they can check your details to ensure you are eligible.
3. Submit the following documents
If you are eligible, you will need to then submit the following documents:
Aadhaar Card
Ration Card
Income Certificate (if required)
Caste Certificate (if applicable)
4. Application and verification
The officer will process your application, verify the documents and complete the Ayushman registration.
5. Download to receive your card
When the verification has been successfully completed, your Ayushman card will be generated. The CSC officer will then download your card and hand it over to you.
